Joining the multi-disciplinary team

 

Anyone can join the Health Centre. If you have a health card, (entitlement from the Department of Employment & Social Security depends on residency on the Island for a period of six months or more) or if you do not hold a health card (full cost of your consultation will be charged) you are still welcome to register.

 

Please download the Primary Registration Form (adult) - Primary Registration Form (child) - or Registration Form for Secondary Services (ie Visitor, Travel, Dermatology) Or Sports Medicine Registration Form which must be completed fully before you can be put on our list. (If you are an existing patient and need to amend/update your details please download the Amendment Form) You will be registered with the Health Centre but may state a preference to see a particular Doctor or member of the multi-disciplinary team. Once the form is completed, please return it to the surgery by post to:

If you would like to register to book appointments online please click here and return the form to the practice. This service can be offered to Primary Registered Patients only.

DUE TO CORONAVIRUS ALL ONLINE BOOKING IS CURRENTLY SUSPENDED UNTIL FURTHER NOTICE. 

We apologise for any inconvenience caused.

 

We offer all new patients a basic new patient registration appointment, which is chargeable. The new patient check can include:

 

  • Height/Weight/BMI

  • Blood pressure

  • Review of past medical history

  • Review of family medical history

  • Review of current medication

 

You do not have to be registered with Health Plus Doctors to see other members of the Health Plus team.

 

 

WE ARE HERE TO HELP YOU.

OUR AIM IS TO BE POLITE AND HELPFUL TO ALL PATIENTS.

HOWEVER, SHOUTING AND SWEARING AT PRACTICE STAFF WILL NOT BE TOLERATED UNDER ANY CIRCUMSTANCES AND PATIENTS WHO ARE ABUSIVE MAY BE REMOVED FROM OUR PATIENT LIST.
