Please make your request well in advance.  If you have not been seen for some time your Doctor may request you make an appointment to review your medication before further repeats may be issued.  There is a £5.00 charge for a prescription. Prescriptions will need to be collected from reception. To order a repeat prescription please contact the surgery on 01534 733 322 or fill out the submission form below. (Prescriptions can no longer be faxed to pharmacies.)

When submitting a prescription online you must leave as much detail as possible. All the fields below must be completed and you need to be very accurate regarding the name and strength of medication required.

GP's are only able to prescribe one month at a time unless you have a face-to-face or telephone consultation.
