Please make your request well in advance and allow 48 hours for processing.

Repeat prescriptions can be obtained via the below options.

​

1) Phone the surgery on 01534 733322 and order through reception, ideally after 11am.

​

2) Submit the online form at the bottom of this page*

​

3) Email your request directly to healthplus@healthplus.gpnet.je (please ensure you state the exact name and medication requested AND the dose - also including the patients name and DOB)

4) If you have registered for online access (Patient Access) repeat prescriptions can be made directly - sign up to Patient Access here

 

If you require an urgent or same-day prescription this can be arranged only by phoning the surgery on 733322.

 * When using the form below you must leave as much detail as possible. All the fields must be completed and you need to be very accurate regarding the name and strength of medication required.

​

Doctors are obliged to carry out periodic blood tests and reviews. If your prescription is outside the review date you may be provided with 1 month's supply to tide you over pending arranging your consultation/review. Only medications listed on your repeat prescribing can be requested here, if any items were prescribed as 'acute' previously, they may require a GP consultation and your request may be declined. Usually, 1 month's medicine will be issued, unless the doctor has previously authorised a three-month prescription for repeat and no further checks are necessary for the safe on-going prescribing. Please note that three months is the maximum you may request without seeing the doctor.

There is a £5.00 charge for a 1 month prescription. Prescriptions will need to be collected from reception, or we are able to post prescriptions to a pharmacy of your choice for a £10.00 charge.
